Veterinary Referral Form

In the state of Texas, a Veterinarian Referral Form must be completed and signed by both the pet owner and the veterinarian for your pet to receive chiropractic care. This ensures that chiropractic treatment is appropriate for your pet's condition and aligns with their overall healthcare plan.

 

If a pet is not a suitable candidate for animal chiropractic care, or if further diagnostics are required from their veterinarian before treatment, we will not proceed with care and will refer the pet back to their veterinarian as needed.
